Communities4Change (C4C) Wales is an evidence informed, time limited approach which brings together individuals from multiple agencies with a common goal to enable and accelerate change to improve health and health equity. Please see Blueprint document for further details on the approach.
The approach was piloted with Cwm Taf Morgannwg Healthy Housing Partnership who were established to improve the health and well-being of the people in Rhondda Cynon Taf, Merthyr Tydfil and Bridgend Local Authorities through improving the quality of housing and level of support to those who are homeless. The partnership’s goal for the pilot was to develop a mechanism for sharing, interpreting and using data for action across partners in relation to housing and health.
A process and impact evaluation of the pilot was undertaken by the Wider Determinants of Health team with support from Research and Evaluation Division in Public Health Wales. The evaluation approach, results and learning from pilot can be found in the report and supplementary annex below.
