Has your organisation got an idea they want to take forward with partners but need support to make it happen?

If the answer to these questions is yes, this is where the Communities4Change Wales approach could support you to achieve your goal.

Communities4Change Wales is an evidence informed, time limited approach which brings together individuals from multiple agencies with a common goal to enable and accelerate change to improve health and health equity. Public Health Network Cymru and the Wider Determinants of Health team in Public Health Wales are seeking to test and evaluate this approach by the end of March 2022.

Your organisation will receive support and online facilitation / coaching and guidance from members of our team. The approach is designed to accelerate change and would require an action phase of up to 3-months and a follow-up phase which would be undertaken by our team. We will also be evaluating the approach with support from an external agency.  For details of the approach see our blueprint.

We are asking interested individuals to complete a short application form which will be considered against the criteria. It is anticipated the preparation phase for community will start in mid to late September.
