Communities4Change (C4C) Wales is an evidence informed, time limited approach which brings together individuals from multiple agencies with a common goal to enable and accelerate change to improve health and health equity. Public Health Network Cymru and the Wider Determinants of Health team in Public Health Wales are seeking to test and evaluate this approach by the end of March 2022.
Following an expression of interest, we are excited to announce that Cwm Taf Morgannwg Healthy Housing Partnership are participating in the pilot of the approach. Cwm Taf Morgannwg Healthy Housing Partnership is a multi-agency group which aims to improve the health and well-being of the people in Rhondda Cynon Taf, Merthyr Tydfil and Bridgend Local Authorities through improving the quality of housing and level of support to those who are homeless.
A senior leadership group has been established and the group have attended preparation workshops hosted by the C4C Wales facilitation team. These workshops have focused on identifying the goal for the C4C Wales collaborative, who needs to part of the collaborative and what steps need to be completed to achieve the goal by the end of the 100 day action phase. The senior leadership group have agreed that the collaborative will focus on developing a mechanism for sharing, interpreting and using data for action across partners in relation to housing and health. It is expected that the 100 day action phase will start in mid-December.
